The treatment of heel fat pad syndrome differs significantly from the treatment of plantar fasciosis because the problem is not inflammation or degeneration of the plantar fascia. Instead, the objective is to protect the heel's natural shock absorber, reduce repetitive impact through the calcaneus and address the factors that have contributed to deterioration of the fat pad.

The first priority is reducing excessive mechanical loading. This does not necessarily mean complete rest, but it often involves temporarily modifying activities that repeatedly compress the heel. Long periods of standing, running on hard surfaces and high-impact exercise may need to be reduced while symptoms settle. Most patients can remain active by substituting lower-impact activities such as cycling, swimming or cross-training during the recovery period.

Footwear is one of the most important treatments. Well-cushioned shoes help absorb impact before it reaches the heel, reducing the forces transmitted through the damaged fat pad. In contrast, walking barefoot on hard floors or wearing minimalist footwear often aggravates symptoms because the already compromised fat pad must absorb even greater forces. For many patients, simply changing to more supportive and better-cushioned footwear produces meaningful improvement.

Heel cushions and shock-absorbing inserts are commonly recommended. Unlike orthotics designed primarily to alter foot biomechanics, heel cushions aim to replace some of the cushioning that the damaged fat pad can no longer provide. Silicone heel cups, gel heel pads and other shock-absorbing devices may reduce local pressure beneath the calcaneus and improve comfort during walking. The choice depends on the patient's footwear, activity level and the underlying cause of the symptoms.

Orthotic therapy may also play a role, particularly when abnormal foot mechanics are contributing to excessive loading of the heel. If excessive pronation, altered gait or poor pressure distribution is identified during assessment, orthoses may help redistribute forces more evenly across the foot. However, orthotics should not be viewed as treatment for the fat pad itself. Their purpose is to reduce the mechanical overload contributing to the condition.

Taping techniques may provide temporary benefit in selected patients. Certain taping methods aim to centralise the heel fat pad beneath the calcaneus, helping maintain its position during walking. Although the scientific evidence supporting fat pad taping remains limited, some patients report meaningful short-term symptom relief, particularly during periods of increased activity.

Rehabilitation remains an important part of management. Strengthening the muscles of the foot, ankle and lower limb may improve overall biomechanics and reduce repetitive loading through the heel. Flexibility exercises, balance training and progressive strengthening should be tailored to the individual rather than following a standard programme designed for plantar fasciosis.

Body weight may also influence symptoms. While heel fat pad syndrome certainly affects people of all body sizes, reducing excessive body weight where appropriate decreases the load transmitted through the heel with every step. Even modest reductions in weight may produce meaningful reductions in heel loading over thousands of steps each day.

Unlike plantar fasciosis, treatments such as shockwave therapy, prolotherapy and platelet-rich plasma have very limited evidence for isolated heel fat pad syndrome. At present, there is insufficient high-quality research to recommend these treatments routinely. Their role may evolve as further research becomes available, but current management continues to focus primarily on mechanical protection of the heel.

Surgery is rarely indicated. Unlike many tendon or nerve disorders, the heel fat pad itself cannot easily be reconstructed or restored surgically. Management therefore focuses on preserving the remaining fat pad, improving shock absorption and modifying the mechanical factors responsible for ongoing overload. When symptoms persist despite appropriate treatment, the diagnosis should be reviewed to ensure that another condition such as plantar fasciosis, Baxter's nerve entrapment or calcaneal bone marrow oedema has not been overlooked.

Recovery is often gradual. Because the fat pad has a limited capacity for structural regeneration, the aim is usually to improve function, reduce symptoms and prevent further deterioration rather than expecting complete restoration of the original anatomy. With an accurate diagnosis and appropriate management, however, many patients experience substantial improvements in comfort and activity levels.

The most important principle is that heel fat pad syndrome is primarily a mechanical problem. Successful treatment depends on protecting the heel from excessive impact rather than repeatedly treating it as though it were plantar fasciitis.
