Heel fat pad syndrome can usually be diagnosed by combining a careful clinical assessment with appropriate imaging. The challenge is that many patients are initially diagnosed with plantar fasciitis because pain beneath the heel is commonly assumed to originate from the plantar fascia. A thorough assessment is therefore essential to determine whether the pain is arising from the heel fat pad, the plantar fascia, the heel bone or several structures simultaneously.

The consultation begins with a detailed history. Understanding exactly where the pain is located, when it occurs, what activities aggravate it and which treatments have already been attempted often provides important clues. Patients with heel fat pad syndrome frequently describe pain beneath the centre of the heel that becomes progressively worse with prolonged standing or walking on hard surfaces. Many report that cushioned footwear provides more relief than stretching exercises.

The physical examination then focuses on identifying the exact source of the pain. Direct pressure beneath the centre of the heel often reproduces symptoms, whereas tenderness at the attachment of the plantar fascia may be relatively mild or absent. The clinician also assesses foot posture, gait, ankle movement, footwear wear patterns and any biomechanical factors that may contribute to excessive loading of the heel.

An important part of the examination is determining whether other conditions are also present. Heel fat pad syndrome frequently coexists with plantar fasciosis, calcaneal bone marrow oedema or Baxter's nerve entrapment. Each condition requires a different management strategy, so identifying every clinically significant pain generator is essential. Successful treatment often depends on recognising that more than one diagnosis may be contributing to the patient's symptoms.

Diagnostic ultrasound has become one of the most valuable investigations for heel fat pad syndrome. Ultrasound allows the thickness and internal structure of the heel fat pad to be assessed while also examining the plantar fascia during the same appointment. A healthy heel fat pad demonstrates a characteristic organised appearance, whereas degeneration, thinning or disruption of the fibrous septae may indicate fat pad pathology. Measurements can also be compared with the opposite foot when only one heel is affected.

One advantage of ultrasound is that it provides dynamic, real-time assessment. The fat pad can be examined while the patient changes foot position or bears weight, allowing the clinician to evaluate how the tissues behave under different loading conditions. This information cannot be obtained from static imaging alone.

MRI may be recommended when the diagnosis remains uncertain or when additional pathology is suspected. MRI provides excellent visualisation of the heel fat pad and surrounding structures, including the plantar fascia, calcaneus, tendons and nerves. It is particularly useful when calcaneal bone marrow oedema, stress fracture, inflammatory arthritis or other deeper pathology is being considered.

Plain X-rays are usually of limited value for diagnosing heel fat pad syndrome because they do not demonstrate the soft tissue structure of the fat pad. They may occasionally identify associated bony abnormalities, but a normal X-ray does not exclude fat pad pathology.

One of the most common mistakes is diagnosing heel fat pad syndrome solely because the patient has central heel pain. Pain location alone is insufficient. The diagnosis should always be supported by a combination of history, clinical examination and imaging findings. Similarly, reduced fat pad thickness on ultrasound does not automatically explain the patient's symptoms unless the clinical presentation is consistent.

Ultimately, the diagnosis is about identifying the true source of the pain rather than simply identifying an abnormal scan. By combining clinical assessment with diagnostic ultrasound and MRI where appropriate, it becomes possible to distinguish heel fat pad syndrome from plantar fasciosis and other common causes of chronic heel pain. This ensures that treatment is directed towards the correct structure and avoids unnecessary or ineffective therapies.
