More often, it is because the underlying pathology has been misidentified or the condition has progressed beyond the reach of simple mechanical stimulation. Shockwave therapy is designed to stimulate a healing response by causing controlled micro-trauma. However, if the fascia has undergone significant degenerative change (fasciosis), the mechanical "thumping" of shockwave may not be enough to restart the biological healing engine. In these cases, I often look toward Ultrasound-guided regenerative injections, which can more directly address the internal structural failure of the fascia.

Once we have identified that a case is truly recalcitrant, my approach shifts to more advanced, evidence-based medical and surgical options. We move away from the "wait and see" model of conservative care and towards targeted intervention.

1. Ultrasound-Guided Hydrodissection

For confirmed cases of Baxter's Nerve Entrapment, Ultrasound-guided hydrodissection is a highly effective, minimally invasive procedure.
