One of the most common questions I hear from patients is, 'Why hasn't my plantar fascia healed?' Most soft tissue injuries improve with time, but the plantar fascia is exposed to thousands of loading cycles every day. When the stress placed upon the tissue continually exceeds its ability to recover, healing may stall and chronic plantar fasciosis can develop.
Healing depends on a balance between tissue damage and tissue repair. Normally, microscopic injury stimulates the body to produce new collagen and restore tissue strength. When excessive mechanical loading continues, the repair process cannot keep pace. Instead of producing healthy, organised collagen, the fascia develops disorganised fibres, degenerative change and reduced mechanical strength.
There is rarely a single reason why healing fails. Common contributing factors include prolonged standing, sudden increases in exercise, poor recovery between activities, reduced calf flexibility, weakness of the foot and lower limb muscles, altered biomechanics, obesity, smoking, diabetes and increasing age. Often several of these factors occur together, creating a cycle of repeated overload.
It is also important to understand that pain relief does not necessarily mean healing has occurred. Symptoms may improve after rest, supportive footwear or temporary treatments, but if the underlying mechanical overload remains unchanged, pain often returns as activity increases. Long-term recovery depends on restoring the tissue's ability to tolerate normal loading rather than simply reducing pain.
Depending on the diagnosis, treatment may include education, progressive rehabilitation, footwear modification, orthotic therapy, ultrasound-guided regenerative injections such as prolotherapy or platelet-rich plasma (PRP), ultrasound-guided hydrodissection when nerve involvement is present and, for carefully selected patients, minimally invasive surgery after comprehensive conservative management has been exhausted.
The objective is not simply to relieve pain today. It is to restore the plantar fascia's capacity to heal, tolerate normal daily activities and reduce the risk of symptoms recurring in the future.
