Plantar fasciosis is the most common cause of chronic pain beneath the heel, but its symptoms can vary considerably from one person to another. While many patients experience the classic pattern of first-step pain in the morning, others describe aching after prolonged standing, pain during exercise or discomfort that gradually worsens throughout the day. Understanding these symptom patterns helps guide diagnosis, but no single symptom is diagnostic on its own.
The hallmark symptom is pain beneath the inside of the heel where the plantar fascia attaches to the calcaneus. Many patients describe sharp discomfort when taking their first few steps out of bed or after standing up following a period of rest. As the tissue warms up, symptoms often improve, only to return after prolonged walking, standing or sporting activity.
As plantar fasciosis progresses, patients may notice that recovery takes longer after exercise and that activities they previously tolerated become increasingly painful. Some describe a constant dull ache, while others experience intermittent sharp pain when the fascia is suddenly loaded. The severity of symptoms does not always reflect the degree of tissue degeneration seen on imaging.
Although these symptoms are common, they are not unique to plantar fasciosis. Baxter's nerve entrapment, heel fat pad syndrome, calcaneal bone marrow oedema, stress fractures and inflammatory arthritis may produce similar pain patterns. Some patients also have more than one condition contributing to their symptoms, which is why an accurate diagnosis requires more than recognising a typical symptom.
Recognising the symptoms early is important because chronic plantar fasciosis often responds better when contributing factors such as excessive loading, reduced strength, footwear issues and biomechanical abnormalities are addressed before the condition becomes more advanced. Treatment is individualised and may include rehabilitation, footwear advice, orthotic therapy, ultrasound-guided regenerative injections such as prolotherapy or platelet-rich plasma (PRP), ultrasound-guided hydrodissection if nerve involvement is identified, and minimally invasive surgery for carefully selected patients after comprehensive non-surgical care has been exhausted.
The key message is that persistent heel pain should never be dismissed as 'just plantar fasciitis'. Understanding the pattern of symptoms is the first step towards identifying the true cause and selecting the most appropriate treatment.
