Every time we take a step, the forces travelling through the foot are greater than our actual body weight. Research suggests that relatively modest reductions in body weight may substantially decrease the forces transmitted through the foot during every step. Because walking involves thousands of steps each day, even small reductions in loading accumulate over time and may lessen stress on the plantar fascia and other weight-bearing structures.

Weight management should never be viewed as a standalone treatment for heel pain. Simply losing weight does not correct Baxter's nerve entrapment, repair a torn plantar fascia or resolve tarsal tunnel syndrome. Instead, healthy weight management should be considered one component of a comprehensive treatment programme that also addresses the underlying diagnosis, rehabilitation, footwear and activity modification.

Although no specific diet has been proven to cure chronic heel pain, good nutrition supports the body's natural repair processes.

It is also worth recognising that heel pain can make weight management more difficult. Patients often reduce their physical activity because walking becomes uncomfortable, leading to reduced energy expenditure and potential weight gain. This creates a frustrating cycle in which pain limits activity, reduced activity contributes to weight gain and additional weight increases mechanical stress on the painful tissues. Breaking this cycle often requires modifying exercise rather than stopping it altogether.

Healthcare professionals increasingly recognise that weight management should be discussed in a supportive, individualised and non-judgemental manner. Every patient's circumstances are different, and sustainable lifestyle changes are generally far more successful than short-term restrictive diets. The objective is long-term improvement in overall health rather than simply achieving a number on the scales.

It is equally important to remember that many patients with chronic heel pain are already at a healthy body weight. In these individuals, factors such as training errors, foot biomechanics, occupational demands, nerve entrapment or underlying medical conditions may play a much greater role than body weight. Treatment should therefore always focus on the individual rather than assuming every case of heel pain is weight-related.

Published research consistently supports maintaining a healthy body weight as part of a broader strategy for reducing the risk of chronic heel pain and improving long-term outcomes. However, the evidence also makes it clear that body weight is only one piece of a much larger puzzle. Successful treatment requires identifying and addressing all of the contributing factors unique to each patient.
