Bunion surgery is commonly performed and can improve pain, alignment and footwear tolerance in appropriately selected patients. However, it is still surgery, and an informed decision requires a balanced discussion of potential benefits, limitations and complications.
General surgical risks include infection, wound-healing problems, bleeding, blood clots and reactions to medication or anaesthesia. Individual risk varies according to health, circulation, smoking or nicotine exposure, medications and the operation performed.
Procedure-specific risks include delayed bone healing, non-union, movement or irritation from fixation, recurrence of the deformity, overcorrection, stiffness, persistent pain, altered sensation or nerve irritation. Transfer pain beneath the lesser metatarsals can occur if forefoot loading remains unbalanced.
Swelling is expected and may persist for several months. The scar can remain sensitive or firm for a period of time. The big-toe joint may not regain completely normal movement, particularly if stiffness or arthritis existed before surgery.
Recurrence does not always mean that an operation was performed incorrectly. Hallux valgus is influenced by anatomy, mechanics, deformity severity, soft-tissue balance and postoperative factors. Careful procedure selection and correction are intended to reduce risk, but no operation can guarantee that the toe will remain perfectly straight for life.
Realistic goals are reduction of bunion-related pain, improved alignment, better function and a wider choice of comfortable footwear. Surgery cannot guarantee unrestricted use of narrow fashion shoes, a completely invisible scar or a foot that looks identical to the other side.
The balance of benefit and risk is most favourable when the bunion is genuinely symptomatic, the diagnosis is clear, the procedure matches the deformity and the patient can follow the recovery plan. A considered decision is preferable to either rushing into surgery or avoiding assessment because of outdated assumptions about bunion operations.
