Modern imaging has transformed the assessment of chronic heel pain, but it also has the potential to be misunderstood. Many patients believe that the scan itself provides the diagnosis. In reality, imaging is only one part of the diagnostic process. The patient's history, physical examination and imaging findings must all agree before a confident diagnosis can be made.
Ultrasound, MRI and X-rays each provide valuable information, but each has limitations. An ultrasound may demonstrate plantar fascia thickening, an MRI may identify bone marrow oedema and an X-ray may reveal a heel spur. However, none of these findings automatically explains why a patient is experiencing pain. Structural changes can be present in people who have no symptoms at all.
Similarly, some patients experience significant pain despite relatively subtle imaging findings. Pain is influenced not only by tissue changes but also by mechanical loading, nerve irritation, rehabilitation status and, in longstanding cases, changes within the nervous system. This is one reason two patients with similar scans can have very different symptoms.
For this reason, I use imaging to answer specific clinical questions rather than to replace clinical reasoning. If the examination suggests plantar fasciosis, ultrasound may confirm degeneration within the plantar fascia. If deeper bone pathology is suspected, MRI may identify stress injury or bone marrow oedema. If arthritis or bony alignment needs assessment, X-rays may be appropriate. Each investigation is selected because it contributes information that influences management.
Good clinical practice involves correlating every imaging finding with the patient's symptoms. If an abnormality is seen on a scan but does not match the location or behaviour of the pain, it may be incidental rather than clinically important. Conversely, imaging may appear relatively normal even though the patient's symptoms are genuine and require treatment.
The most successful treatment plans are built by combining all available information into a single diagnosis. Imaging should increase diagnostic confidence rather than create confusion. Used appropriately, it helps ensure treatment is directed at the true source of pain instead of simply treating an abnormality reported on a scan.
